Investment: $2.50 - $4.00 PMPM

Impact: High-volume Social Health data and Red Flag Detection.

Tier II: Clinical Engagement
(High-Frequency)

Purpose: Focused Support for Moderate-Risk Members.

Service: 2-3 Proactive Weekly Emotional and Conversational Check-In Call (15-30 min) + Monthly Social Health Narrative Reports.

Investment: $15 - 35 PMPM

Impact: Reduce ‘Nuisance’ Calls to Clinics and increase Medication Adherence.
